The excimer laser is an exciting medical instrument uniquely suited
to the task of refractive corneal surgery. In all types of
refractive surgery, the cornea is reshaped so that the rays of light
are focused clearly on the retina. This laser produces a cool,
ultraviolet beam of light which literally vaporizes tissue as it
breaks carbon-carbon bonds. Tissue is removed in a precise fashion
on a microscopic level. Each pulse of the laser removes 0.25 microns
of tissue or 1/39 millionth of an inch. This allows the cornea to be
reshaped gently and precisely into a more desirable shape.
LASIK (Laser Assisted in-Situ Keratomileusis)
is a two step procedure. First, the surgeon creates a micro-thin
corneal flap using an ultra-fast, femtosecond laser that uses an
infrared light beam. The flap is attached by a “hinge” and folded
back from the rest of the cornea. In the second step, an excimer
laser is used to reshape the cornea. The laser treatment takes place
in the corneal bed and the flap is replaced in its original
position. The flap is guided into position by the eye’s natural
suction ability. The procedure takes less than 10 minutes and is
painless.
PRK (Photorefractive Keratectomy)
is a procedure in which the front surface of the cornea is directly
sculpted by the excimer laser. The surgeon prepares the eye by
gently removing the surface layer of the eye known as the
epithelium. This layer regenerates itself within a few days.
Computer controlled pulses of the laser are directed onto the
surface to reshape the cornea. Less than 10% of the cornea is
affected with the deeper layers remaining virtually untouched. The
whole procedure takes less than ten minutes and is painless.

LASIK surgery is a common surgical procedure to correct
nearsightedness, farsightedness and astigmatism. Using the excimer
laser, or cold laser, the doctor removes the damaged tissue beneath
the corneal flap with very little or no scarring. The entire
procedure from start to finish takes a matter of minutes. Patients
have reported clearer vision within a few hours following the
surgical procedure.
In LASIK, a small flap is made on the surface of the cornea. A
suction ring will hold the eye in place. You will not see anything
or feel any pain, but you may feel some pressure at this time. When
the flap is turned over, your vision will become a little blurry.
You will be asked to fixate on a red target light. The laser part of
the procedure is performed in 20-60 seconds.
The flap is then placed back into position and the surgeon will wait
3 to 5 minutes for the eye to create a natural vacuum to hold the
flap down. The typical LASIK procedure takes about 10 minutes per
eye.
Once the surgery is complete, you will be able to go home almost
immediately. First, we place some additional drops into your eyes
and instruct you on using your medications.

Hawaii's first PRK laser procedure and Dr. Jenkins the first LASIK.

Laser Vision Correction
To decide whether LASIK is right for you, a consultation with our
refractive surgeon and a preliminary screening are required.
Measurements should be taken of your pupil size and corneal
thickness, and corneal mapping should be done during your
preliminary screening.
Before LASIK eye surgery, a complete eye examination must be
performed. A discussion should also take place with our LASIK
surgeon that includes your goals and expectations, the procedure's
risks and benefits, and the doctor's assessment of the anticipated
outcome of your LASIK eye surgery. Dr. Alan Faulkner will also
advise you on what sort of precautions and steps you need to take
following the LASIK procedure. Ultimately, we want you to be
comfortable with Dr. Faulkner and have a
realistic expectation of what LASIK can do for you before deciding
to undergo the eye surgery.
Immediately after LASIK is complete, your vision will be slightly
blurry. Post-operative instructions include wearing protective
shields for one week, wearing protective goggles for six months to a
year if you participate in contact sports, avoiding swimming or hot
tubs or getting dirty water in your eyes for one week, avoiding
rubbing your eyes, and using eye drops for five to seven days.
After LASIK, minor eye discomfort may be experienced, and it is best
to sleep for several hours after surgery, or at least rest with your
eyes closed. Most patients are able to drive themselves to their
next day post-operative check up and return to work following their
appointment. Other routine post-operative examinations are performed
at one week, one month, three months, six months, and one year after
LASIK to check your visual acuity and perform other tests as needed.
The importance of routine post-operative care should not be
underestimated. Although rare, complications can occur, and early
detection and intervention can minimize their impact.
